The Office of the Registrar keeps
and updates the students' records
pertinent to academic performance
and requirements, courses offered
and member of units given to a
subject in accordance with the
requirements prescribed by the
Department of Education, Commission
on Higher Education and Technical
Education and Skills Development
Authority.
The following students' records are
requested at the Registrar's Office:
Transcript of Records
Certification of units earned/CAR
Special Order
Summer Permit
Cross enrolment permit
Honorable Dismissal/ Transfer
Credential
Form 137-A
Form 138
Diploma
Procedures for Servicing Student's
Request
1. Secures Clearance/
Request Form at the Registrar's
Office Service Window 7 and
fills up data needed in the
Clearance/ Request Form and
indicates what record is being
requested.
2. Student process
signing of Clearance/ Request Form
by the following :
A. For First Request
- Dean/ Principal
- Librarian
- Assessment
- Treasurer
- VP Finance
B. For Subsequent Request
(Second and succeeding request)
- Assessment
- Treasurer
- VP Finance
3. Proceeds to Service
Window 8 (Record Section) and
presents the accomplished
Clearance/ Request Form. Record
Custodian pulls out student's
envelope and advises the student to
proceed to Service Window 7.
4. If the student's
record is complete and intact, the
Staff in-charge gives the
assessment of the fees for the
requested records and advises the
student to pay to the Cashier.
5. After payment submit
receipt to Registrar's Office
Service Window 7 for claim
slip of requested record/s.
Upon compliance of the requirements,
requested records will be claimed at
least one (1) week from the date the
request is received by the Office of
the Registrar.
