To receive financial assistance, you
must be fully admitted to a
degree program or be enrolled in a
qualified certificate program.
To participate and be afforded
financial customization and/ or
support, fill up Financial Support
Program Application Form
(downloadable), and indicate your
desired financial support program or
service.
Qualifying requisites for students
and parents/ guardians
1. For students -
must be officially enrolled and must
not be a candidate for
dropout
2. For parents/
guardians - must have regular source
of income (daily,
weekly, bi-monthly or monthly)
3. If guardian is
not related (by affinity) or
consanguinity) to the
student, a perpetual
deed of support must be executed in
favor
of the students.
Basic requirements for programs that

participation
1. Photocopy of
COR and School ID
2. Photocopy of
Parents'/ Guardian's acceptable ID
(with picture and
signature)
3. Accomplished
Application Form for program desired
to participate
4. Request for
servicing payment proposal (form
provided)
5. If employed,
certificate of regular employment
and salary
certification, payslips
(at least four regular payroll
cycles)
6. For retail
stores/outlets - certificate of
business registration or
certification issued by
LGU executive head (barangay,
municipal or city)
7. For Retail
Service - certificate of training by
government-
recognized
agencies/institutions or
certification issued by LGU
executive head (barangay,
municipal or city)
Process and stages to participate
1. Secure and fill
up Financial Support Program
application form from
Financial Aid Office (or download
here»)
2. Comply
requirements
3. Submit
requirements to the Financial Aid
Office
4. Wait for field
visitation and discuss with
representatives for your
preferred scheme and
serving arrangements.
5. Upon advice,
visit Financial Aid Office for final
orientation, discussion
of terms and conditions and signing
documents.
6. Receipt of
documents signed and start of
preferred
program/service.
